How can we know we are truly hearing from God?

Answered in 1 source

We know we are hearing from God when we are drawn to Him and respond to His call (1 Samuel 3:10).

The assurance of hearing from God is rooted in the transformative experience of being drawn to Him. In 1 Samuel 3:10, Samuel answers the Lord's call, 'Speak, for Your servant hears.' This signifies a posture of readiness and openness to God's voice. Recognizing that God speaks through His Word and the preaching of the Gospel is fundamental. When believers feel compelled to seek Him, experience conviction of sin, and desire to know Him more intimately, it is evidence that they are hearing from God. This relationship is characterized by ongoing communication through prayer, Scripture reading, and the sacraments, affirming one's place in the family of God.
