How do we know that Jesus is worthy to open the book?

The worthiness of Christ is established in Revelation 5 through His role as the sacrificial Lamb. Unlike any other, He has triumphed over sin and death by His willing sacrifice. The contrast in the passage highlights that no one else is worthy—a truth evidenced by the inability of men or angels to open the scroll.
